Dear Veteran,
If you have been scheduled for a disability evaluation, welcome.  I am excited to meet you.  I have included some Frequently Asked Questions below for your information.

Sincerely,
Kristin Ober, Ph.D.

Veteran FAQ

What do I need to bring to my evaluation?

Please bring a list of your current medications.  Also, you can bring a list of your mental health symptoms if you feel that would be helpful to you.

Why am I scheduled for this appointment?

If you have filed a VA claim for a mental health condition, you have been referred to me for an evaluation of that condition.

What if I need to reschedule?

Please contact VES directly at 1-877-637-8387.

How do I get there?

My office is located on the South side of Tulsa.  If you are taking the Creek Turnpike, you will exit on Yale and turn North towards 91st street.  Go North through the intersection at 91st and Yale.  You will see a Bancfirst on your left.  The driveway to my office is directly after the Bancfirst.  It is a long driveway up to a building that looks like a house.  I am in Suite S.  Go all the way back to my office.  If the door is closed, I am meeting with another Veteran and will be with you in a moment.  There is a waiting room near my office.  If the door is open, please come in.

What will we discuss during my examination?

We will discuss your history (pre-military, during military, and post-military), your symptoms, and how your symptoms impact you.

Can I bring my spouse, adult child, friend, etc?

You can bring whomever makes you feel comfortable.
